The Downloadable Forms screen allows you to view, save and/or print forms SCERA has made available to you.
To access the Online Forms screen, click on the Online Forms option in the left menu. The Online Forms screen appears.
Downloadable Forms: Clicking on the form opens the document. Note that you will need the ability to open Adobe Acrobat (.pdf) documents. Once the form is open, you can then save it locally or print it. Some basic information, such as Name or Address, may be pre-populated on the form.
Note that Adobe Reader is required to view or print forms because they are generated as a PDF documents. A link is provided to obtain a free download of Adobe Reader if needed. In addition, this screen provides a link to the Adobe Reader Verification page, which verifies that you have a supported version of Adobe Reader installed, and provides links to Adobe user documentation.
